I witnessed this one. Root cause was a lower rear wishbone failure.

I think it may have been Le Mans or Indy 500.

Crashes involving fan fatalities were common in the 50's and 60's, until better fencing was added to tracks to make them safer for all concerned. The worst was at Le Mans in 1955, when one of the Mercedes team cars went into the crowd at well over 100 mph, killing over 80 spectators. Mercedes left racing for nearly 30 years afterwards.

Michael McDowell in qualifying at Texas in 2008, anyone who watched this happen won't forget it (I was watching a dubious stream back in the days when justin.tv still existed since we only get the races here in the UK, not any of the practice or qualifying)

He didn't exactly "walk away" from this.
But maybe it's a minor miracle he's still walking. [Happily he's 100% recovered.]
