That picture has been around for years. Unfortunately I do not have the original, but it was shown that the pic here is a clever bit of manipulation to make it look further in than was the case.

Nice find. I don't think they are the same shot. I enlarged the smaller one and tried to line them up. They won't line up at all.
First I lined up the borders. When that didn't work I ignored the boarders and tried to line up the hand and opposing thumb. Nothing lines up. Here's an overlay. Also if that one pic is a "Photoshop" job its damn good. I really don't see any cloning going on there. So my guess is the "x-rated" shot was an outtake and never used in the magazine. Eventually the whole set was released online.

I think this confirms it: picture 3 is the fake that Staffsyeoman is referring to that has been debunked, and picture 2 is the original of that. I'm sure we'll all feel happier now.
