Unseen but Man to Man Yearbook, 1975-Fall lists a Sally Coleson [sic] as the cf, see here.

A Sally is also listed on the cover of Man to Man, 1975-10. /shrug/

The Modern Man shoot is by Larry Caye but I don't see any reason not to use this name.

Here's a slide scan. Note the mole below her breasts which can also be seen in the TLI catalog scans and in the Debonair August 1976 mag scan.
