Its a lot of things I think. Partly it is a religious based stigma (the US is still a significantly puritanical country) and there are various cliques based on religion, both mainstream and not so mainstream. *cough*Scientology*cough* Not to mention the flat out evangelical pressure groups trying to force Hollywood productions to strictly follow their own interpretation of <insert religious text of choice here>. They'll organize boycotts of movies and TV shows as well as of the companies that place products in them or buy commercial time on them just for showing too much cleavage. How do you think they react to casting former porn stars? Which leads to the next part....

Partly I suspect its Hollywood's severely risk-adverse culture. Casting a known porn star is a potential public relations disaster and when the stupidest tiny things can already derail a hundred million dollar project studio execs are rarely willing to add any additional possible problems. They drop A-list actors from blockbusters or hit TV series for getting traffic tickets or going to the wrong nightclub. With that kind of thinking by studios its something of a wonder that former sex workers get even bit parts.
