I haven't had these particular P&H issues, but I do get occasional screwed-up posts in which there are unintended dupes, things are out of order, etc. I agree that it's tedious in edit-post mode to edit the code itself, but if you click that A/A thingy in the upper right corner to switch modes, it will show your images instead of code. Then click on an image to "box" it, and then you can delete it, etc.
