Is any one having problems with the uploader?

Everytime I try and upload any images I find 2 problems.

1. If I select 20 images to upload it asks me if I want to insert 234 for example

2. I uploaded some images and a few of them are duplicated in my post.
I know I can go in and remove them but this is time consuming.

I want to continue uploading when I can as it's only fair as I download occasionaly, but this is becoming frustrating.
