Sometimes upscaling and re-encoding can be warranted. If resolution is below 512x384 it is hard to see details and I have also seen enough of good quality rips ruined by a poor deinterlacer. If you have the dvd source it's better to re-encode the whole thing but if not, proper deinterlacing can improve an otherwise good quality rip. But these are rare exceptions.
